Being i...I wish we had read this like one year ago. Being in a rural practice this whole EHR deal is misery come to roost at home! I have often repeated, ad nauseam, that quote from the classic film “Cool Hand Luke”: “What we have here gentlemen, is a failure to communicate.” Therein lies the major problem I fear; more so in a day and age where “documentation” seems to be given precedence over “doctoring.” I am of the old school belief that a “high-touch” approach trumps a “high-tech” methodology when it comes to patient care, if high-tech is the current state of the EHR systems available. To Quote Alfred Doolittle from “My Fair Lady”:<br />“Who asked him to make a gentleman out of me? I was happy. I was free. I touched pretty nigh everyone for money when I wanted it, same as I touched him. Now, I'm tied neck and heels, and everybody touches me. A year ago, I hadn't a relation in the world except one or two who wouldn't speak to me. Now, I've fifty, and not a decent week's wages amongst the lot of 'em. You know I believe in technology, but good technology should be there to help the user first and foremost not the ACO, or whatever else comes next. If the user can do a great job, the ACO will take care of itself, IMHO.<br />I want to see the certification juggernaut stopped and I want to see EHR vendors go back to developing stuff based on user input and requests. All the cool stuff that everybody wants to do, but never has time (or resources) to do anymore, because they have to keep up with regulations. <br />There will be very little (to none) true innovation in health IT until the regulators stop thinking that there is only one solution to every problem, and that they know exactly what that solution is. Most of that article is spot-on and defines the EHR challenges in 2014... even though the article is more than 2 years old. Little of the needed change and progress that Shah defined has come to pass. The over-reliance on flawed EHRs as being the solution needed is a main reason that 75% of ACOs are failing. Those that aren't tend to have focused on I.T. basics and good patient care in a decentralized, supportive fashion with subsequent, more practical aggregations of data rather than trying to attempt granular interoperability at this point.<br /><br />"Theoretically, you, and all other onesies twosies, or even twenties and fifties, who form the (still) independent majority, could stand up and stop the madness. Theoretically."<br /><br />Who is going to thrive in the future? Will it be those starting to stand up and who are demanding an end to the madness? But to recognize interdependence and act on it requires an attitude of solidarity which we're constantly told on radio and TV is socialism-lite, and lite eeeeevil is still eeeevil. So our Independent Individualists refuse to work together, to share, to engage at all really in the sort of solidarity pointed at by the mostly-discarded Hippocratic Oath.
