Trying to give them the benefit of the doubt as best I can :-) There will be lots of fights down this road.Margalit Gur-Ariehttps://www.blogger.com/profile/08777722834145614546noreply@blogger.comtag:blogger.com,1999:blog-3503957686158274288.post-27237310226255791092015-05-08T10:51:48.583-05:002015-05-08T10:51:48.583-05:00Great commentary.I was naturally not surprised tha...Great commentary.I was naturally not surprised that ACP and AMA were big supporters of MACRA, but I was a bit surprised and disappointed that many (most?) local and state medical associations did as well. I guess they thought that SGR was so bad that anything would be better. I wonder how long before they realize they were very wrong.james gaultehttps://www.blogger.com/profile/05537303135780186926noreply@blogger.comtag:blogger.com,1999:blog-3503957686158274288.post-83643855593272360052015-05-07T17:40:17.395-05:002015-05-07T17:40:17.395-05:00Thanks, Dr. B. They somehow think that capitation will provide more money and/or reduce administrative costs. Other than DPC practices, I have yet to see how capitation increases payment, or reduces administrative burdens. To me it sounds like jumping from the fire to the frying pan.
IMHO, the starting point should be to double (and perhaps triple) the current rates for all primary care, so it covers all the value-added activities that are necessary beyond face to face visits, sit back for a while, and then measure "outcomes" all you want.... :-)
As an addendum, and as long as I am dreaming, it would also help to force hospital corporations to divest themselves of all primary care physicians they are currently holding.
